睡眠時無呼吸症と高血圧

いびきや日中の眠気などで知られる「睡眠時無呼吸症」。

交通事故との関連で、目にすることがありますが、最近、高血圧を始めとした生活習慣病や脳卒中、心不全といった脳心血管系疾患との関連性についての解明がすすんできています。

とくに、高血圧や心不全の原因疾患になることが明らかになり、北米における高血圧のガイドライン(JNC-7)でも、高血圧、また治療抵抗性高血圧(複数の降圧剤投与でも、難治性の高血圧)の最も一般的な原因疾患として、とりあげられています。日本高血圧ガイドラインでは、2009年から、高血圧の原因疾患として、睡眠時無呼吸症の存在が明記されはじめました。

また、心不全の原因疾患にもなることがわかり、アメリカ心臓病学会では心不全の原因疾患として、睡眠時無呼吸症をあげています。既に心不全を発症している人についても、睡眠時無呼吸症の合併が予後のリスクファクターのひとつであることがわかり、その診断や積極的治療に注目が集まっています。

専門領域では、このように一般的になりつつありますが、すべての医療者の間で十分に認識されているとはいえません。

実際に、次のような事例がありました。

50歳男性、肥満はなし(body mass index 23.0 kg/m2)。

大学病院で高血圧の専門医にかかっており、5種類の降圧剤を内服しても、なかなか血圧が下がらない状況。

たまたま見た週刊誌で、無呼吸症との関連を知り、外来を受診された。

確定診断のための終夜睡眠ポリソムノグラム検査(一泊入院)をおこなったところ、1時間あたりの無呼吸低呼吸指数(AHI)が60回/時で、重度の無呼吸症と診断された。

そこで、持続陽圧呼吸療法(CPAP)をおこなった結果、無呼吸症の改善にともない、血圧が低下し、降圧剤を減量することができた。

潜在的に、同じような患者さんがとても多く存在していることでしょう。

現在、私たちは、全国の睡眠検査施設で、「高血圧や心血管系疾患と睡眠時無呼吸症の関係をみる多施設前向き研究:SCCS http://j-sccs.net/」をおこなっています。

日本人において、しっかりしとしたエビデンスをつくり、ひとりでも多くの方の「生活の質」(QOL)を豊かにすることができればと考えています。


Sleep Apnea Syndrome and Hypertension

Sleep Apnea Syndrome is well known for its symptoms such as loud snoring and daytime sleepiness. This disease is often related with traffic accidents. Recently, the relationship with cardio- cerebrovascular diseases such as hypertension, lifestyle diseases, stroke and heart failure is starting to be revealed.

It has been reported that sleep apnea causes hypertension and heart failure and North American hypertension guidelines (JNC 7) listed sleep apnea as one of the common causes for both hypertension and resistant hypertension (uncontrolled blood pressure despite the use of multiple antihypertensive medications). In Japan, sleep apnea was first listed a cause of hypertension in Japanese Hypertension Guideline in 2009

Also, it was discovered that sleep apnea causes heart failure which led American Heart Association to acknowledge sleep apnea as a cause of heart failure as well. As for patients with heart failure, it is reported that complication with sleep apnea would be a risk factor for mortality. Therefore, diagnosing and treating sleep apnea actively is a crucial topic among medical professionals. 

These facts about sleep apnea has been well acknowledged among sleep specialists, but unfortunately, not among all medical professionals

Here is an example.

50 years old, male, non-obese (body mass index 23.0 kg/m2)
He has been seeing a hypertension specialist at a university hospital and prescribed five different medications. Despite of these medications, his hypertension could not be controlled. One day, he learned about the relationship between hypertension and sleep apnea in an article of a magazine and came to my office. He went through an overnight polysomnography for diagnosis and turned out that his apnea-hypopnea index (AHI) was 60, which indicates sever sleep apnea syndrome. He then started his CPAP treatment and as a result, his blood pressure improved as his sleep apnea was controlled and was able to reduce the number of prescribed medication.

I believe that there are many potential patients just like him. Currently, we are conducting a study "The Sleep and Cardiovascular Disease National Prospective Cohort Study (SCCS) http://j-sccs.net/ " among sleep clinics and hospitals throughout Japan. I hope this study would provide solid scientific evidence for Japanese and help us improve quality of life (QOL) for our nation.
